Chronic Care Management

Our Chronic Care Management (CCM) program provides ongoing support to help patients better manage chronic health conditions. Through this service, you have access to our care team 24 hours a day, 7 days a week for assistance and guidance related to your treatment plan.

Our CCM services include:

• Continuous assessment and monitoring by our care team

• Support with scheduling and receiving recommended preventive services

• Medication monitoring and oversight

• Development and maintenance of personalized care plans

• Coordination and communication with other healthcare providers when necessary

• Direct access to our Chronic Care Management team at 843-405-4645

As part of the program, we will:

• Clearly explain the available services and how they support your care

• Provide updates and access to your care plan as needed

• Notify you if your participation in these services changes or is discontinued

By enrolling in Chronic Care Management services, you agree to:

• Receive CCM services and allow us to bill Medicare or other applicable insurance providers for these services

• Authorize electronic communication between our office and other physicians, hospitals, or healthcare providers involved in your care

• Be responsible for any applicable charges if you do not have insurance or if your deductible or coinsurance requirements have not been met
