Chronic Care Management

Our Chronic Care Management (CCM) services provide you with 24/7 access to our practice:

• Ongoing assessment by our team to access your care
• Plans to help you receive needed preventive services
• Medication oversight
• Ongoing plans for your care
• Interaction with other doctors when needed
• Direct line to our Chronic Care Management team (843) 405-4645

We will:

• Explain these services to you
• Provide you with your plan of care as needed
• Notify you if these services are going to be discontinued

You agree to:

• Receive these services, allowing us to bill Medicare and any other insurance you may have for these services
• Authorize us to communicate electronically with other doctors, hospitals, and other caregivers about your care
• Be responsible for these charges if you have no insurance, or if you have not reached your annual deductible or have a co-insurance due
