What Are Your Options for Prostate Cancer Treatment?

When a man is diagnosed with prostate cancer, there are several treatment options available. The most appropriate approach depends on factors such as the stage and aggressiveness of the cancer, overall health, and personal preferences. Understanding these options can help patients make informed decisions about their care.

Active Surveillance

Active surveillance is often recommended for men with low-risk, slow-growing prostate cancer. Instead of starting treatment right away, the cancer is carefully monitored through regular PSA blood tests, digital rectal exams, and sometimes repeat biopsies. Treatment is only initiated if the cancer begins to show signs of progression.

Surgery (Prostatectomy)

Surgical removal of the prostate gland, known as a prostatectomy, is a common treatment for prostate cancer that is confined to the prostate. This procedure can be performed using traditional open surgery, laparoscopic techniques, or robotic-assisted methods. The goal of surgery is to remove the cancer completely and is often recommended for men who are otherwise healthy and expected to live many more years.

Radiation Therapy

Radiation therapy uses high-energy radiation to destroy cancer cells. It may be delivered externally through external beam radiation therapy (EBRT) or internally through brachytherapy, in which small radioactive seeds are placed directly into the prostate. Radiation therapy can be used as a primary treatment or after surgery if there is concern about remaining cancer cells.

Hormone Therapy

Hormone therapy, also called androgen deprivation therapy (ADT), works by lowering the levels of male hormones that stimulate prostate cancer growth. This treatment is often used for advanced or recurrent prostate cancer and may be combined with other treatments such as radiation therapy.

Chemotherapy

Chemotherapy uses medications that target rapidly dividing cancer cells. It is typically used when prostate cancer has spread beyond the prostate or when other treatments are no longer effective. Chemotherapy can help slow cancer growth, relieve symptoms, and improve quality of life.

Immunotherapy

Immunotherapy helps the body’s immune system recognize and attack cancer cells. One example is sipuleucel-T (Provenge), an FDA-approved treatment used primarily for certain advanced cases of prostate cancer.

Targeted Therapy

Targeted therapies focus on specific genetic mutations or proteins that contribute to cancer growth. For example, PARP inhibitors may be used in prostate cancers associated with certain inherited mutations such as BRCA1 or BRCA2.
